In Silico Validation of MCID Platform for Monte Carlo-Based Voxel Dosimetry Applied to 90Y-Radioembolization of Liver Malignancies

The aim was the validation of a platform for internal dosimetry, named MCID, based on patient-specific images and direct Monte Carlo (MC) simulations, radioembolization liver tumors with 90Y-labeled microspheres. CT real patients were used to create voxelized phantoms different density activity maps. SPECT acquisitions simulated by SIMIND MC code. Input macros GATE/Geant4 code generated loading coregistered morphological functional performing image segmentation. dosimetric results obtained from simulations conventional MIRD approach at both organ voxel level, in condition homogeneous tissues, compared, obtaining differences about 0.3% within 3%, respectively, whereas increased (up 14%) introducing tissue heterogeneities phantoms. Mean absorbed dose spherical regions sizes (10 mm ≤ r 30 mm) OLINDA/EXM also compared varying range 7–69%, which decreased 2–9% after correcting partial volume effects (PVEs) imaging, confirming that mostly due PVEs, even though still high difference smallest sphere suggested possible source description mismatching. This study validated MCID platform, allows fast implementation GATE simulation, avoiding complex time-consuming manual coding. It points out relevance personalized accounting inhomogeneities, order avoid misestimations.
